Substitute Process FY 2011-2012
If you would like to be a substitute in SAU 70 for a
Teacher, Educational Assistant, Receptionist or Nurse, print and complete the forms below, and call Carol Edwards (643-6050 x 2006) to make an appointment. To become
a teacher substitute you must have four (4) years of college
or a BA. If you do not have these credentials, you will be a probationary
substitute and must substitute three (3) times at one school and
be approved by the principal.

Substitute Application
I-9: verification employment (To save paper, print only Page 4 of form.) You must present
a PASSPORT; OR DRIVER'S LICENSE and SOCIAL SECURITY CARD; OR
DRIVER'S LICENSE and BIRTH CERTIFICATE
at the time of appointment. (See the I-9 form for other acceptable documents.) You can type in the form online, then print.
W-4 Tax Form (federal) You can type in the form online, then print.
W-4 Tax form - VT (both must be completed
if you reside in Vermont)
Confidentiality Form
Child Abuse Form (informational only)
Reimbursement Form This form allows you
after substituting 5 times in either district to be reimbursed for
the criminal background check.
Besides these forms, please provide a résumé and 3 names with complete mailing addresses and e-mail addresses. Your references will receive a form to complete.
Pay
Teacher Substitute - $70.00 per day for 4 hrs or more
Educational Assistant Substitute - $60.00 per day for 4 hrs or more
Nurse Substitutes - must have RN License for corresponding state - $125.00/day
Costs for background checks
New Hampshire schools: $20 to Hanover Police, $51.50 check payable to "State of NH - Criminal Records."
For Marion Cross School: $41.50 check payable to "Norwich School District."

NOTE REGARDING TEACHING/CERTIFIED POSITIONS: To be considered for a teaching or other certified position with SAU 70, you must have the proper state certification or a letter from the corresponding State Department of Education confirming that an alternative plan is in place. Your applications should include:
- Letter of Interest

- Copy of current certification or confirmation you are in-process for
the appropriate state certification. You must confirm that you meet HQT qualifications.
