IMPORTANT HEALTH COVERAGE TAX DOCUMENTS
Notice of Availability of IRS Form 1095 (Employer-Provided Health Insurance Offer and Coverage).
To reduce postage costs, paper waste and streamline administrative processes, we will no longer automatically print and mail paper copies of this form to your home address. This change in practice aligns with the Federal Paperwork Burden Reduction Act, which gives employers the flexibility to provide these forms digitally or upon direct request.
Please note that Form 1095-C is not required to file your federal income tax return. It is simply a statement verifying the health insurance coverage that was offered to you during the prior tax year.
How to get your copy:
If you would like a paper or electronic copy of your Form 1095-C for your records, you can request one at any time. We will fulfill all requests within 30 days. Please send your request to Human Resources:
